The application form is one of the most important documents that we need to submit when applying DJID certification. It has been being used when the Directorate General name was PARPOSTEL to SDPPI, and now the latest one is DJID. We must prepare and fill it carefully as it has huge impact towards the certification process.
This document contains the applicant information and the product information including product’s name, trademark, model name or number, technical standard reference, frequency operation and the output power.
When the certification system was still offline, we need to submit this application in hardcopy, the earlier application form even contains the factory information such as factory name, factory address and the country of origin. In this time frame, the application form name is “FR.PM5”.
Starting 2018 online system was used and the updated application form was also used. It eliminates some information in the application form as the factory information data is inputted in the online submission website. In this time frame, the application form name is “Deklarasi Kesesuaian” (declaration of conformity).
And now, DJID has announced the latest format for the application form and it also eliminates the frequency range information, output power information and the technical standard reference, the additional information is only the “ID Aplikasi” (application ID). This ID is obtained when we draft the application in DJID website.
For your information, this latest declaration has been being used from Augst 1, 2025 then it becomes fully mandatory on September 1, 2025. So, in this writing, we will show you the application form changes from time to time:
