When processing Indonesia Type Approval, there are many documents or data required to be submitted through the online portal of Directorate General of Digital Infrastructure (DJID). To make you understand about what these documents are, here are the list of required documents that have to be submitted mandatorily to DJID. The list is divided into three sections, based on the type of application that can be choose: Full Local Testing, Paperwork, and end product paperwork based on module’s RF report.

Full local testing

  1. Specification Sheet that mentions brand and model applied
  2. Photos of the product
  3. User manual
  4. RF Test SOP and Test Software
  5. Antenna gain information [value, in dB]
  6. Application form PM. 5
  7. MoU/Distributor agreement between brand holder and Applicant / Local Representative, signed by both parties
  8. Applicant / Local Representative information [company name, contact person name, email address, and contact number]
  9. Conformity declaration signed by applicant [Required upon test completion]

Paperwork application

  1. Product Specification/Brochure that mentions model name
  2. Product Photos
  3. RF report from SDPPI-recognized test Lab.
  4. Summary of the RF test report signed by the lab who issued the RF report [Dimulti will arrange the draft after reports are received]
  5. EMC test report from SDPPI-recognized test Lab.
  6. CB test report from SDPPI-accredited test lab. Note: this is only required if the product is supplied by AC power, or when the product must be continuously supplied by AC/DC adapter.
  7. Authorization letter report owner to Applicant/local representative to use their reports
  8. Application form PM. 5
  9. Conformity declaration signed by Applicant / Local Representative [Dimulti will arrange the draft]
  10. MoU/Distributor agreement between brand holder and Applicant / Local Representative, signed by both parties
  11. Conformity declaration signed by Applicant / Local Representative
  12. Applicant / Local Representative information [company name, contact person name, email address, and contact number]

Paperwork Application using module’s RF reports

  1. Product Specification/Brochure that mentions model name
  2. RF test report from module
  3. Summary of the RF test report signed by the lab who issued the RF report [Dimulti will arrange the draft after reports are received]
  4. EMC test report from end product
  5. If the product uses brick adaptor or AC power, we need CB test report from end product
    • Note: all submitted reports have to be issued by SDPPI-recognized test lab
  6. Authorization letter from each test report owner to Applicant / Local Representative to use their test reports
  7. Block diagram
  8. Circuit diagram
  9. End product photos
  10. Module photos
  11. Photos showing the module is installed in the end product
  12. Declaration letter of containing module signed by manufacturer
  13. Application forms PM5
  14. MoU/Distributor agreement between end product brand holder and Applicant / Local Representative, signed by both parties
  15. Conformity declaration signed by Applicant / Local Representative
  16. Applicant / Local Representative information [company name, contact person name, email address, and contact number]
